1. First, call Hope Psychological Services at 781-402-2442 ext. 4 in the greater Boston area, or 508-787-1048 ext. 4 in the Metro West area, to schedule an initial appointment.

2. After you have scheduled an appointment, please print the form below that your therapist requested you to complete. You may bring it to your first appointment, or fax it to our confidential fax at 781-402-1744.
